unopposed alpha adrenoceptor activation:
when beta receptors are blocked, the extra catecholamines that can't bind to beta will bind to alpha receptors causing increased stimulation of alpha receptors. stimulation of alpha causes visceral vasconstriction (visceral means including the heart) so it exacerbates PV disease. (PV disease = vasoconstriction in areas excluding heart)
